Donor Stock Instructions
- If your broker holds the stock in "book" form we prefer that you ask your broker
to wire the stock to our broker at Wachovia Securities. Your broker should use the DTC and transfer the stocks to:

Please ask your broker to phone Rhonda Bissell at (818) 833-6432 before initiating the wire transfer. She will notify the Wachovia Securities broker that the gift is coming. The 'date of gift' is the date that the stocks are transferred into the Guide Dogs of America account. We are unable to use the date notified as the donor still has possession of the stocks up to the time that it is received into the Guide Dogs of America brokerage account.
- If you hold the stock in certificate form, you may do one of two things:
• If the certificate represents the correct number of shares, you may send the certificate, as is, to:

In a SEPARATE ENVELOPE, send a signed stock power and letter of instruction, (i.e., that Guide Dogs of America is to accept the stock as a gift, and outlining any restrictions you may have.) The date of the gift is the postmark date. Stock powers are available through a bank or from Rhonda Bissell at Guide Dogs of America.
- If you wish to give only part of the certificate, you should transfer the desired number of shares into the name of Guide Dogs of America, Tax ID# 95-1586088 and send the new certificate to the address shown above. The date of the gift is the date the stock transferred into the name of Guide Dogs of America, which is the date that is stamped on the certificate.
