2. Please telephone us at (909) 607-3676 or (800) GIVE-2-CMC, or FAX us at (909) 621-8443, with the name of the stock, the number of shares you are donating, the date that the instructions were given to the broker, and your gift instructions (account, uses, etc.).
   
   
3. We will issue a gift receipt for the mean price of the shares on the date they are received in the College's account.

When the stock is registered in your name:

1. Copy the back of the certificate(s).
   
   
2. Sign the copy of the certificate back only, leaving all other information blank.
   
   
3. Mail the certificates to CMC in one envelope. Our mailing address is:

4. Mail the signed copy of the certificate back to CMC in a separate envelope.
